  • You have indicated that you are not a desk-based worker, therefore you do not need to complete this assessment. 

     

    If this is not correct, or you would like to complete the assessment anyways, please change your answer to the question above to "yes".

  • Instructions

  • On commencement of employment for desk-based roles, one checklist should be completed for each workstation, i.e. office and home.

    If you have an adjustable height desk, start with Section 1 – Chair Setup and adjust the desk to the chair height and continue to Section 2 - Workstation Setup; or
    If you have a fixed height desk, start with Section 2 – Workstation Setup, and continue to Section 1 - Chair Setup to adjust the chair to suit the desk height.

  • 1. Workstation Details

  • Team members who work from home are required to provide their own equipment, unless specified otherwise in an employment contract. Please confirm that you have the following equipment before proceeding.*
  • 2. Chair Setup

  • Please work through the following checklist. 

  • I am familiar with all the adjustment mechanisms on my chair*
  • I have checked all adjustment mechanisms are in good working order*
  • The back support is at an angle of 90-100 degrees to the seat pan*
  • I can sit right back in the chair, so the back support fits neatly into the hollow of my back and there is approx. 2-3 finger width clearance between the front edge of the seat and the back of my knee*
  • Where fitted, chair arms are low enough to clear the workstation*
  • When seated with hands on the keyboard please confirm you can achieve the following posture (refer to image):

  • 1. Shoulders relaxed and symmetrical with head in midline position*
  • 2. Elbows slightly away from the body and slightly higher than wrists*
  • 3. Wrists in functional position (slightly extended at 10-20 degrees)*
  • 4. Hips aligned and approximately 90-degree bend in knees*
  • 5. Thighs are not in contact with under-surface of desk*
  • 6. Feet flat on the floor or footrest (not dangling)*
  • 3. Workstation Setup

  • Please work through the following checklist.

  • The desk is lower than your elbow when it has 90-degree angle while typing (this applies for both seated and standing work)*
  • There is sufficient leg room when seated (items not stored under the desk)*
  • When using only a laptop with no additional monitor, the laptop is placed on a laptop riser / stand*
  • The top of the monitor / screen is at the same height as your eyes. NOTE:- If wearing multi focal lenses it should be lower or higher to avoid extending the neck depending upon the multi focal part of the glass*
  • If two monitors / screens are used equally, position them symmetrically in front of you at the same height*
  • The external keyboard and mouse are on the same level and directly in front of you, 10 to 15 centimetres away from the edge of the desk*
  • Your wrists are slightly extended (10-20 degrees) whilst keying and not resting on the desk creating a sharp angle at the wrist joint*
  • Your mouse comfortably fits your hand and works freely*
  • Your mouse is positioned close to the keyboard*
  • A headset (over ear) is available for prolonged or frequent phone conversations or if simultaneous keying / writing is required*
  • Frequently used items are within comfortable reach*
  • The monitor / screen is free from glare or reflections and adjusted to a comfortable level of brightness, contrast, font size*
  • The lighting is adequate – there is not need to squint and there is no glare*
  • There are no trip hazards (e.g. cabling, mats, poor housekeeping)*
  • Power outlets and boards are safely set up (i.e. not overloaded)*
  • Electrical equipment in good condition and not damaged*

  • There is a clear and unobstructed access to an exit in case of emergency*
  • A phone is available for contacting emergency services*
  • The work area has sufficient ventilation and is a comfortable temperature (regardless of season)*
  • The work area is secure, private, and allows for confidential conversations (if this is a requirement of your role)*
  • The work area is free from excessive noise and distraction*
